Item1453: Too hard to create new switchboard entries, doc is poor

The switchboard is defined as a hash of arrays, which makes it hard to work out what all the fields are for. AFAICT there's no reason for not using a hash instead, which would be self-documenting as well as making it easier to add fields specific to different actions.
