Item9461: TOC no longer works with an ampersand (trunk)

This used to work on 1.0. See Sandbox.TestTopic44559, in which all headings are working fine.

Then the same topic on trunk, the headings with an ampersand do not work.

Since this is a regression, I set this as urgent. However, I can understand if you changed it to normal.

Also, I see that TOC anchors do not work for headings split across multiple lines with backslash-newline. Like this:
