Item13200: "Insert link to attachment" dialog is NOP

The changes to the Upload dialog appear to have broken the "Insert link to attachment" function.

No JS errors, but after selecting an attachment, pressing Insert link is a NOP.

• Click paperclip to manage attachments
• Choose upload tab and upload an attachment
  • Note that the message about successful upload includes un-rendered html markup.
• Click the "Insert link to attachment" tab
• Select attachment and press "Insert link"
  • Nothing happens - no errors, no web network activity.

Note that the Edit/Insert image dialog works fine. It's only the insert from the Attachment dialog that's broken.
